Choose an upright backrest with arms if you read or watch TV sitting for a while; choose a triangular wedge if you want a simpler, firmer slope you can reposition; choose a small pad only for a localized gap. Measure your shoulder width and the space between the mattress and headboard. A reading pillow supports sitting up—it is not a replacement for a sleep-position pillow.
Two ordinary sleeping pillows can get you upright for five minutes, then slide into a stack behind your shoulders. Reading pillows are made for a different job: giving your back a stable surface while you sit. The right type depends less on a marketing claim of "ergonomic" and more on where you read, how much room you have and whether you want places to rest your arms.

Measure before buying
- Shoulder width: an armed backrest should let your arms fall comfortably rather than squeezing them inward. Measure from shoulder to shoulder while sitting naturally.
- Backrest height: check whether you need support just to mid-back or up to the shoulders and head. A separate head pillow may be more adjustable than a built-in headrest.
- Mattress width: a wide pillow with arms can crowd a twin bed or a partner's side of a queen bed.
- Storage depth: decide where the pillow goes during sleep. A large upright backrest does not fold away like a regular bed pillow.
Firmness and fill
Memory foam tends to hold a shaped backrest; fiberfill or PP-cotton fill can feel softer and more squeezable but may need fluffing. A sponge-filled compact pad is useful for a small gap but will not support the whole back. None of these fills is a medical treatment or guaranteed to feel comfortable for everyone.
Check whether the cover removes for washing. A reading pillow used on a sofa or in bed picks up oils and dust, and a non-removable cover can be awkward to clean. Follow the care label on the actual product rather than assuming the fill can go in a washer.
Position it so it works
- Place the lower edge behind the lower back or against the headboard, not only behind the shoulder blades.
- Sit back rather than hunching forward to reach a book. A lap pillow or light stand can bring the book up to you.
- If the bed is soft, put a firmer cushion under your hips so the backrest does not fold under you.
- Change position and take short movement breaks. A pillow can make sitting more comfortable but does not replace movement.
For a side-sleeping head pillow later in the night, start over with your normal pillow-height check; the reading backrest belongs off the bed or to the side when you lie down.
